ENROLL STUDENTS - WELCOME TO PUSD
ENROLLMENT REQUIREMENTS
- A certified copy of the pupil’s birth certificate; or other reliable proof of the pupil’s identity and age AND a notarized Affidavit of Birth explaining the inability to provide a copy of the birth certificate (A.R.S. §15-828)
- Current immunization records
- Proof of residency (Arizona Department of Education Arizona Residency Guidelines and Affidavit of Shared Residence Form)
- Parent/Guardian state-issued Photo ID (or equivalent legal form of I.D. per Governing Board Policy JFAA)
- Withdrawal Form (If previously enrolled in an Arizona school)
- Guardianship/Custody Documentation (if applicable)

Need to update your student’s information? Use the PUSD Student Information Change Form. Fill out this form and email it to central.registrar@pusd.org (supporting documentation may be required) or bring it to the Central Registrar at the District Office. 902 W.
